The TPL and Medicare Payer IDs spreadsheet lists out, in table format, the following key information:

• Payer ID
• Payer Name
• Payer Status
• From and To Dates

Note: Carriers who do not have a Payer ID on the list can enter “99999” in the Payer ID field.
